Guillain-Barre’ syndrome (GBS) is an acute autoimmune polyneuropathy.Several types of peripheral neuropathy are associated with human immunodeficiency virus (HIV) infection.GBS occurs most often at the time of seroconversion or in the early course of infections where the CD4 count is relatively preserved.Occasionally,GBS is reported in advanced stage of acquired immunodeficiency syndrome (AIDS) with CD4 count less than 50 cells/mm3 or during immune reconstitution.Here,we studied a unique case of GBS that occurred after an advanced stage of AIDS with a CD4 count of 3 cells/mm3 and high titer IgG anti-sulfatide antibodies.To the best of our knowledge,this is the lowest CD4 count reported in HIV-GBS.Serum sample was tested for IgM and IgG antibodies against brain ganglioside mixture,LM(l) ganglioside; sulfated glycolipids,sulfatide and sulfoglucuronyly paragloboside (SGPG) using an ELISA and a thin-layer chromatogram (TLC)-overlay technique.
