As reported, Decapitated and Decaudated Spermatozoa (DDS) was one of sperm abnormality. Most previous studies focused on the ultrastructure of the sperm morphology by electron microscopy. However, rare reports uncovering the pathogenesis and genetic mechanism of DDS defects were published. A rare case of a patient with the Decapitatedand Decaudated Spermatozoa (DDS) which belonged to the severe teratozoospermia was reported and its potential genetic cause was discovered in this study. Sequencing the HOOK1 gene related to DDS revealed a novel non一synonymous mutation of A toG (p.Q286R). This new gene mutation in the central coiled-coil domain could increase fragility of the head-tail connection of the spermatoza, supporting the important role of a DDS defect in the pathogenesis and genetic mechanism of spermiogenesis defect.
