Neuropathy is the single most important feature responsible for foot ulcerations in people with diabetes.The aim of this presentation is to discuss the musculoskeletal and biomechanical changes associated with neuropathy that occur in the diabetic foot.Metabolic and vascular factors can cause hypoxia leading to a reduction in nerve conduction and velocity.The alteration in large fibre nerve conduction and velocity leads to poor proprioceptive quality and small muscle wasting, ultimately affecting the sensorimotor function of the foot.Hyperglycaemia mediated synthesis of new collagen and accumulation of non-enzymatic end products speed up age-related changes in skin, connective tissue, joints and tendons.
