Management of high-risk prostate cancer (PCa) is still controversial.Aim of this study was to report outcome of radical prostatectomy (RP) or high-dose-rate brachytherapy (HDR) for the patients with localized high-risk PCa in Jikei University Hospital.Between November 2003 and November 2010, a total of 592 high-risk PCa underwent RP (331) and HDR (281).Most of HDR cases were performed with external body radiation therapy and long term androgen deprivation therapy.The definition of the biochemical recurrence (BCR) of each treatment differs as to that prostate specific antigen (PSA) ≥ 0.2 ng/mL for RP, PSA nadir plus 2 ng/mL for HDR.The biochemical recurrence free survival (BCRFS) was retrospectively investigated and compared between the modalities.
