Background :Blepharoplasty has become one of the most common esthetic procedures performed today.The protruding fat and lid-cheek junction are the most conspicuous aging changes that need to dealt with.During the procedure of oral approach zygomatic reduction, it was found occasionally that the orbital fat can be exposed through the perforated periosteum at the inferior obital rim.Thus, A new blepharoplasty procedure through oral approach was explored and developed, and described in detail in this paper.Methods :Seventeen patients aged from 26 to 38 were involved in this study, including six, with a previous unsuccessful blepharoplasty and one, with a history of lower lid injury.The procedure was performed under the infraorbital nerve block and local anesthesia.An intraoral incision at the upper vestibular groove was used.Periosteum was elevated on the surface of the maxilla to the infraorbital rim, with the infraorbital nerve was preserved.The periosteum and the orbital septum were incised along the whole length of the infraorbital rim.The fat that exposed through the incision was either removed or preserved and fixed to the out layer soft tissue with sutures.Results: Cosmetic results were obtained in all the patients with the disappearance of * Manuscript with title (excluding any references to authors, their names or their affi...baggy lower lids and with the natural smooth transition of the Eyelid-Cheek Junction.No visible incision scar, eyelid ectropion, lid and ball separation, epiphora, palpebral fissure dysraphism, and inferior palpebra retraction were found in this group of patients.Oral incision healed without infection.Six cases developed numbness in the infraorbital region, five of them recovered within 3 months, and the other one recovered 6 months postoperatively.Conclnsions:the technique of intraoral approach blepharoplasty could effectively blend lid-cheek junction and treat the orbital fat without visible scar, achieving an ideal aesthetic results.
