Objective: To investigate the relationship between 18F-fluoro-2-deoxy-glucose (18F-FDG) uptake of primary lesions and epidermal growth factor receptor(EGFR) mutations in patients with non-small cell lung cancer (NSCLC) and the feasibility of EGFR mutations predicted by the maximum standard uptake value(SUVmax).Methods: Totally 66 patients dated from Nov.2006 to Sept.2010, who underwent PET / CT examination and EGFR mutation analysis before any anti-tumor therapies, then we did retrospective analysis and research.The 18F-FDG uptake of the primary lesions was described with SUVmax and the EGFR mutation status in exons 19 and 21 were detected by fluorescent quantitative PCR method.Results: The EGFR mutation rate was 33% in exons 19 and 21.The SUVmax in EGFR mutant type was lower than wild type (9.8±4.8, 13.9 ± 5.5,P=0.006).The SUVmax and EGFR mutations were performed as a negative correlation (r =-0.344, P =0.005).For the prediction of EGFR mutations using SUVmax, the area under the receiver operating characteristic curve (AUC) was 0.71(P =0.006), when chose 8.8 as cut-off value, the Youden Index reached a maximum value of 0.43.And also the EGFR mutation was more likely to occur in the patients with SUVmax lower than 8.8 (P=0.000).The SUVmax was still a statistically significant predicter for EGFR mutations in multi-factor logistic regression (P =0.007).Conclusion: The 18F-FDG uptake of primary lesions in NSCLC patients was negatively correlated with EGFR mutations; patients with lower SUVmax were more likely to harbor EGFR mutations.For the NSCLC patient, it might be helpful to identify who is suitable to tyrosine kinase inhibitors (TKI).
