Objective To investigate the effectiveness of the Medication Review Clinic(MRC)to solve drug-related problems(DRPs)for elderly.Methods This study was carried out as a prospective case-series intervention study.A pharmacist-led MRC was conducted to identify and solve DRPs for residents from 6 communities and a senior care center in Chongqing,China.Adults(≥ 65 years)diagnosed with cardiovascular disease or diabetes with either(1)prescribed ≥4 chronic medications or(2)had visited ≥2 different physicians were enrolled to MRC.Participants were followed for 12 months.The DRPs identified and types of recommendation provided during the MRC were classified according to a modified version of the DOCUMENT classification system.Results For 102 clinic participants,the mean age was 69.4 years and they had on average 3.2 chronic conditions(range 1-7)and received a mean of 6.3(range 2–16)different drugs.489 DRPs were recorded.The most frequently identified DRPs categories were: “Undertreated”,followed by “Over-or under-dose” and “Drug selection”.These DRPs led to a total of 526 recommendations.The most frequently recommendations provided were “Add a drug to therapy” followed by “Monitoring: laboratory test”,and 60.89%recommendations made by pharmacists were accepted and implemented by general practitioners.The number of drugs taken by patients was the significant factor with an influence on the frequency DRPs.Conclusion DRPs were frequently observed in elderly and the effectiveness of MRC appears was positive.Clinical pharmacists play a vital role in the optimization of long-term medication treatment for residents.
