Why D2?Surgery plays a key role in curable gastriccancer．Although chemotherapy effects have recentlyincreased bynewly developed powerful agents，no cure can yet beexpected without surgery．It is a global consensusthat goodlocal tumor control is essential for gastric cancertreatment，but the extent of optimal lymphadenectomyis controversial.D2 lymphadenectomy has been the gold standard inJapan．It was established，and still is beingpolished，based on the large database of pathological andsurvival data of patients．
