Herpes zoster is infection of varicella-herpes virus and the mortality is low.Here reported a case of virulent herpes zoster involving ophthalmic branch of trigeminal nerve with encephalitis and vasculitis, testified by autopsy.The patient was an old man and was admired for fever and headache for five days herpes in left frontotempral area for three day unconscious for one day.Four days later, the patient died.The result of autopsy :bleeding and erosion on scalp and necrosis of polyepithelium.Neuronal degeneration and necrosis and perivascular cuffing on both sides of frontal and temporal lobes , small foci of bleeding in the left side of temporal lobe and both sides of insular lobes.cerebellar hematoma.Discussion: Old age is the definitive risk factor of this herpes case.The pathological results revealed not only the existence of neuritis of ophthalmic branch of trigeminal nerve, but also that of vasculopathy, which resulted into cerebellar hemorrhage and death of herniation.
