Abdominal Aortic Surgery: Prevention of Perioperative Cardiac Complication with Mininvasive and Fast

　　Fast track programs have been introduced to improve postoperative outcome(1), also in vascular surgery(2).It deals with programs that have the purpose of optimizing whole perioperative care(3).We have verified the effectiveness of these programs in the cardiologic outcome of aortic abdominal surgery.Our programs found on: perioperative optimization-particularly maintaining cardiologic therapies-included antiplatelet agents; ACC/AHA guidelines adhesion(4); mininvasive surgery; thoracic epidural analgesia; immediate postoperative recovery of ambulation and feeding.Between May 2000 and June 2009 we treated 642 patients for abdominal aortic aneurismectomy.Mean age was 70,4 (±8,2) years.ASA risk (values between 1 and 5) was ≥3 in 84,9% patients.MET index was <4 in 2,3%, between 4 and 7 in 60,3 % and >7 only in 37,4% patients.86 % patients assumed antipltelets drugs.Mean operation time was 157 min (±53).Mean haematic losses were 745 ml (±707).
