Late temporal lobe radiation injury in patients with nasopharyngeal carcinoma:evaluation with diffus

  Objective The objective of this study was to evaluate late temporal lobe radiation injury by multi-section diffusion weighted MR imaging.Methods We prospectively studied 37 patients with typical clinical symptoms of late temporal lobe injury or other abnormalities in the temporal lobes incidentally detected by conventional MR imaging.All patients had a previous history of radiotherapy for histologically proven NPC.Conventional T1-and T2-weighted images, fast gradient echo with echo-planar diffusion-weighted MR imaging were performed.Apparent diffusion coefficient (ADC) map were computed via commercially available software and comparison with contrast group(health people).Results 25 patients had bilateral temporal lobes involvement and 12 patients had unilateral lesion.Finger-like high signal lesions on T2-weighted images were observed in 12 patients.A large cyst like lesion was present in 2 patient.Severe mass effect upon adjacent structures was observed in 12 patients.The temporal lobe lesions displayed marked high diffusion on the ADC map.The average ADC value of temporal lobe white matter was (2.08±0.33)× 103 mm2/s, marked higher than normorl peoples[(0.71±0.08) × 10-3 mm2/s].There is statistical significance between both group.Conclusions We have observed that late temporal lobe injury after radiation treatment consistently had high diffusion on DWI.Diffusion occurred in the majority of patients.On the basis our initial finding, the results may have clinical usefulness in predicting the outcome of treatment.Further prospective study is required.
