Incidence of PCOS is 4-7% in infertile women.In Asia, 1/3 of PCOS is classified as obese type and 2/3 as non-obese type.Insulin resistance and hyperandrogenism are higher in obese type and LI-I/FSH ratio is higher in non-obese type.We recommend weight loss and metformin therapy (lower insulin, LH, and free testosterone levels) in obese patients with PCOS, and oral contraceptive pills (OCP) administered before controlled ovarian stimulation (COS) to decrease LH levels in nonobese patients with PCOS.Pregnancy rates in PCOS are favorable by IUI and IVF compared with other indications.In IUI, rFSH low-dose step-up protocol is better to make a mature unifollicle for a single pregnancy and avoid OHSS.In IVF, GnRH antagonist protocol (with Letrozole) may be a recommended treatment for good quality oocytes and embryos, and may also prevent severe OHSS.IVM for PCOS awaits further investigation, although we have had more than twenty babies following IVM.
