Background:Pyogenic granuloma (PG) is a benign vascular lesion of unknown etiology, it commonly occurs on the head and face.Many different treatments have been used, with different success rates.Cryotherapy is frequent method to treat PG in China.Holmium laser has been using in the uropoiesis calculus and prostatic hypertrophy for near 15 years, and as an alternative method in treatment of genital warts, it had been using in our department for nearly 8 years.We applied this laser in treatment of PG before 3 years also, and have gotten the good results.Objectives: To evaluate and compare the effect and side effect of YAG: holmium laser with cryotherapy in the treatment of PG.Methods: One hundred and forty-one patients with PG were randomized to receive irradiation with holmium laser or cryotherapy with liquid nitrogen.65 patients in the laser group and 57 patients in the cryotherapy group completed the study period.The effect and side-effect were recorded respectively, and the result between two groups was analysis.Results:Among the patients who completed the study, treatment resulted in complete resolution of 63 patients in 65 lesions after immediately one session, only two patients need second session in the holmium group, and after one to four sessions (mean 1.24) in the cryotherapy group.It was 10.77 percent of side-effect (secondary infection, scar and pigmentary change) observed in the holmium laser group, and 22.8 percent of sideeffect were observed in the cryotherapy group by follow-up 3 months.There were statistically significant differences of first clear rate (X2=20.57, P<0.001), and side-effect (X2=7.01, P=0.008) between the two treatment methods.Local recurrence encountered in the 1/65 cases in the holmium laser group, in the 6/57 cases in the cryotherapy group.There was statistically significant difference of recurrence between the two groups.Conclusions:It is better in the patients with PG treated with holmium laser than cryotherapy in the clinical efficiency and side-effect, especially in the cosmetic problems.
