目的探讨有机磷农药中毒救治中机械通气的临床应用价值及关键环节的把握。方法 52例重度口服有机磷中毒患者,对27例就诊时即出现呼吸抑制症状者、25例在治疗过程中出现呼吸衰竭者,分别立即予以紧急和及时气管内插管机械通气治疗;同时即刻给予洗胃、导泻,氯解磷定、阿托品、盐酸戊乙奎醚(长托宁)等特异性药物及其他综合治疗方法;部分重症采用血液透析联合血液灌流。结果 52例患者治愈48例,死亡3例,放弃治疗1例。机械通气时间4~11d,平均6.5d。其中6例发生呼吸机相关性肺炎,1例因气管导管堵塞而重新气管插管,余均未发生机械通气相关并发症。结论机械通气在救治重度有机磷中毒患者中占有非常重要的地位。
Objective To explore the clinical value of mechanical ventilation in the treatment of organophosphorus pesticide poisoning and the grasping of the key links. Methods Fifty-two patients with severe oral organophosphate poisoning were treated with respiratory depression symptoms in 27 cases and respiratory failure in 25 cases during treatment. Immediate and immediate endotracheal intubation mechanical ventilation was given immediately and simultaneously Gastric lavage, catharsis, chlorpromazine, atropine, penehyclidine hydrochloride (penehyclidine hydrochloride) and other specific drugs and other comprehensive treatment; some severe hemodialysis combined with hemoperfusion. Results 52 patients were cured in 48 cases, 3 died, giving up treatment in 1 case. Mechanical ventilation time 4 ~ 11d, an average of 6.5d. Of these, 6 developed ventilator-associated pneumonia and 1 received tracheal intubation due to endotracheal tube blockage, with no complications associated with mechanical ventilation. Conclusion Mechanical ventilation plays a very important role in the treatment of patients with severe organophosphate poisoning.
