目的:通过对大鼠进行8周"电击跳跃法"训练建立大鼠跟腱末端病模型;观察比较火针治疗组与对照组在一次与四次火针治疗后末端区组织的病理变化和末端止点部位IL-6、iNOS指标变化规律,分析探讨火针治疗末端病的机制.方法:1月龄雄性SD大鼠88只,体重110±10克.每笼4只分笼饲养.随机选取8只作为正常组,其余80只作为模型组.模型组大鼠先以"电击跳跃法"进行一天一次的3天适应性训练,每次20 min.正式实验时,在自制造模装置内一次放入8只大鼠,每15s通电一次,持续20 min,休息10 min,再持续20 min.每天造模1次,每周一至周六进行,周日休息,持续造模8周.通电电压由小逐步加大,直到大鼠出现有效跳跃为止(双足跳离铜棒).将模型组随机分为一次火针治疗组与对照组各为4组(分别命名为治疗1、2、3、4组与对照1、2、3、4组),每组8只;四次火针治疗组(1次/3天)与对照组各1组(分别命名为治疗5组和对照5组),每组8只.造模后分别对一次与四次火针治疗组进行施针,每次治疗只施针一次,每日治疗均在同一时间进行.一次火针治疗组施针后于第1、2、3、4天处死治疗组与对应的对照组,并在第1天取材时处死正常组取材,四次火针治疗组在完成第四次治疗两天后进行处死.取大鼠双侧后足整条跟腱和完整的跟腱末端区,包括腱区、腱围、脂肪垫、纤维软骨区、跟骨和跟骨软骨面.将实验样本保存于甲醛固定液中并置于4℃冰箱中保存24小时,其后用清水冲洗3小时置于10%EDTA溶液中脱钙.脱钙完毕后石蜡包埋进行切片,于40℃烘箱中烘三天后于37℃保存,然后进行免疫组化检测.用SPSS13.0统计软件对数据进行处理.测试结果以平均数±标准差()表示,单因素ANOVA方差分析进行组间差异检验,以P<0.05为差异具有显著性水平,P<0.01为差异具有非常显著性水平.结果:白细胞介素-6(IL-6)、诱导型一氧化氮(iNOS)的表达主要出现在跟腱、纤维软骨区、钙化软骨区和跟骨关节面;治疗组与对照组相比各区域的IL-6、iNOS含量总体上都有下降,特别是在治疗4组和5组.治疗4组中,IL-6在纤维软骨区与对照相比下降明显,(P<0.05);跟腱区iNOS的表达显著低于对照组(P<0.05).治疗5组中,IL-6在跟骨关节面下降明显,与对照组相比存在显著性差异(P<0.05); iNOS与对照组相比在跟腱、跟骨关节面存在显著性差异(P<0.05).结论:一次火针治疗可以使末端区IL-6、iNOS得到下调,短时间内达到延缓肿痛的作用,可以认为火针治疗对末端病的治疗达到了缓解炎症的作用,且通过合理增加火针治疗次数效果更佳.
