Longitudinal changes of anatomical brain network during motor recovery after stroke

  Introduction Brain reorganization has been characterized during motor recovery after stroke observed by neuroimaging techniques.Brain white matter regeneration was the basic mechanisms of functional reorganization.In this study,we investigated white matter regeneration and brain reorganization based on complex brain network techniques.Method Eight right-handed patients(5 males; age 68±5 years)with subcortical infarction were recruited in a longitudinal study at the Zhong Da Hospital of Southeast University.All patients were first-onset stroke and manifested motor deficits.They were scanned and clinically assessed at day 1,day 7,day 30 and day 90.In addition,four age-and sex-matched controls were also recruited.We examined all subjects with a 3.0 T MR Siemens scanner.The DTI scheme: 30 diffusion gradients(b = 1000 s/mm2)and 1 non-diffusion-weighted image(b=0 s/mm2),NEX=2.According to the Fugl-Meyer Assessment score(FMS),we defined all patients into two groups: motor function well recovered(MFWR,FMS day 90 – FMS day 0 > 40,4 patients)and no obvious recover(MFNR,FMS day 90 – FMS day 0 < 40,4 patients).Anatomical brain networks were constructed based on AAL labels based on white matter tracking(90 Nodes).Results: Anatomical brain networks were constructed and stroke patients showed significant lower global efficiency(p<0.001).In SMA,there were significant increase of betweenness in MFWR and decrease of degree in MFNR at day 7,while at day 30 there was no significant differences.In putamen,there were significant decrease of betweenness/degree in stroke patients at day 7 compared to day 1 and day 30.Conclusion There were white matter reorganization after stroke and SMA might play an important role during this procedure.Day 7 might be the key time point in brain reorganization.Using SMA as key biomarker,we might be able to predict clinical outcome of stroke patients.
