Objective:TodiscusstraitandtreatmentofunstableprosthesistypeofPFFaftertotalhiparthroplasty. Method:Thestudyenrolled15cases(15hips)withunstablestemofPFFaftertotalhiparthroplasty,including10 malesand5females,andpatientswithfracturearoundacetabulumwasexcludedfromDecember2005to December2014.Twocaseswerecement,thirteenwerecementless,onewashemiarthroplasty,fifteencaseswere changedlongstems,fivecaseswerefixedwithsinglewirerope,fivecaseswerefixedwithwireropecombinedwith allograft,andtheagerangedfrom64to89,withmeanage76.2.ClinicaloutcomewasevaluatedbyHarrisscore andimageologywasevaluatedbyBealsandTowerStandard. Results:Onediedfourmonthsafteroperation.Fourteenwerefollowedmean4.5years(6monthsto8years)and uptolastfollow-uptheHarrisscorewas82.3(50to100).Tencaseswereexcellent,threeweregood,andtwocased werepoorbyBealsandTowerstandard.OnecaseoccurredDeepVeinThrobolismpreoperativelyandtreated withfilterscreen.Twocasesoccurredposteriortibialveinthrombosisrespectivelythreedaysandtencasesdays postoperativelyandwerecuredbyconservativetreatment.Nocomplicationssuchasmalunion,fixation workless,dislocationandprosthesislooseningoccurred. Conclusion:Satisfactorycanbeachievedbyusinglongstemcombingwithappropriatefixtionforunstablestem ofPFF.
