Objective: To explore the value of transoral and transnasal with endoscope combined approach in resection of the upper part of oral tumor.Methods: Clinical data of 8 patients with the tumor in the upper part of oral which invading the lower part of nasal、maxillary sinus or pterygoid process.Primary tumor site : 4 cases in hard platate, 4 cases in superior gum,1 case in the upper part of retromolar tissue.Pathological types :3 cases of adenoid cystic carcinoma,2 cases of mucoepidermoid carcionoma, 2 cases of squamous carcionoma, 1 case of multiform adenoma and 1 case of malignant histiocytoma.Operative procedure:nasal endoscope would be used to treat the horizontal margins of the tumor in the nasal cavity firstly.According to the tumor location,the nasal septum, lateral wall of nasal and lamina perpendicularis of palatine bone were interruptied from piriform aperture in the horizontal direction,while the lacrimal duct could be retained if there is no tumor invasion.The lower part of pterygoid process and its muscle attachments would be cut off after the posterior-lareral wall of maxillary sinus resection.Vertical margins of the tumor are treated via oral cavity.The incisions in the upper part of oral were via plata combining with labium gum, while only plata incisions were performed if the alveolar process could be retained.The primary tumor would be easily resected from oral cavity after all bony connections have been isolated.Oronasal fistula reconstruction with free flap after the tumor resection.
