Objective To Analyse maternal and fetal outcomes in patients with pulmonary hypertension (PH) complicated by congenital heart disease.Material and Methods Historical clinical data of 114 pregnant women with PH complicated by congenital heart disease from Jan 2007 to Jun 2013 were collected.Cases were divided into 3 groups according to the pulmonary arterial pressure: 35 cases of mild PH group (30~49mmHg) , 27 cases of moderate group (50-79mmHg) and 52 cases of severe group (≥80mmHg).The types of congenital heart disease, cardiac function, delivery mode and fetus status were compared.Results The most common types of congenital heart disease in pregnant women were atrial septal defect (ASD, 44.73%) and ventricular septal defect (VSD, 26.32%).The cardiac function of mild PH group was mostly distributed in NYHA class Ⅰ-Ⅱ, while severe group in NYHA Ⅱ-Ⅳ.Cesarean section was the main delivery way.The rate of iatrogenic fatal loss in severe group was obviously higher than other groups, but lower at the rate of term delivery.The difference was statistically significant (P<0.05).Conclusion The rate of heart failure and iatrogenic fatal loss in pregnant women was gradually increased accompany with the severity of PH.The rate of term delivery increased in patient whose congenital heart disease was corrected by cardiac surgery before pregnancy.The unintended pregnancy in cases with severe PH should be terminated as soon as possible.Cesarean is a safer delivery mode for women with PH in second or third trimester.Epidural anesthesia is preferred for surgery.
