目的探讨HPA-1—6、15等位基因与特发性血小板减少性紫癜(ITP)的相关性。方法采用聚合酶链反应序列特异性引物(PCR-SSP)法,对ITP组(n=25)和健康对照组(n=25)进行HPA-1—6、15共7个抗原系统基因分型,采用ELISA法和微柱凝胶法检测2组人群血小板吸附抗体及血小板游离抗体。结果ITP组HPA-3的基因频率明显异于对照组(P<0.01),其中HPA-3 a基因频率高于对照组,HPA-3b基因频率低于对照组,其余等位基因在2组间未发现有明显区别(P>0.05);ITP组血小板游离抗体检测到17例阳性(68%),吸附抗体检出21例(84%),对照组血小板抗体未检出。结论本次研究显示HPA-3抗原系统和ITP的发病可能存在一定的关联;血小板吸附抗体在ITP中检出率明显高于游离抗体。
Objective To investigate the association between HPA-1-6,15 alleles and idiopathic thrombocytopenic purpura (ITP). Methods PCR-SSP method was used to genotype 7 antigen systems of HPA-1-6 and 15 in ITP group (n = 25) and healthy control group (n = 25) , ELISA and microcolumn gel method were used to detect platelet adsorbed antibody and platelet free antibody in two groups. Results The frequency of HPA-3 gene in ITP group was significantly different from that in control group (P <0.01). The frequency of HPA-3 a gene was higher than that of control group, while the frequency of HPA-3b gene was lower than that of control group No significant difference was found between the two groups (P> 0.05). In the ITP group, 17 cases were positive for free platelet antibodies (68%) and 21 cases (84%) for adsorbed antibodies. Platelet antibodies in the control group were not detected. Conclusion This study shows that there may be a correlation between HPA-3 antigen system and ITP. The detection rate of platelet adsorbed antibody in ITP is significantly higher than that of free antibody.
