The influence of target dose uniformity on dose to lung in the treatment plan for non small cell lun

  Purpose:To discuss the influence of target dose uniformity on dose to lung for non-small cell lung cancer(NSCLC)using intensity-modulated radiotherapy(IMRT).Methods:Ten NSCLC patients with right-side lung tumor were selected for this study.Two plans were done for each patient in Pinnacle 9.0.The total dose for planning target volume(PTV)was 60Gy(2Gy/fraction).The plans chose the same beam parameters,and the angles were 185°, 215°, 245°, 345°, 15°, 155°, respectively.One group(G1)plans restrict uniform dose for PTV,while the other(G2)was not.During optimization,keep the dose of cord PRV and heart nearly the same for the two groups.Results:Regarding lung tissues,mean dose,V5,V10,V20,and V30 values were 16.202±2.583 Gy,0.598±0.092,0.440±0.090,0.286±0.048,and 0.202±0.029,respectively,for group G1,whereas those values were 14.480±2.156 Gy,0.536±0.089,0.383±0.081,0.241±0.033,and 0.179±0.025,respectively,for group G2 with a prescribed dose of 60 Gy.The V20 and mean dose of Lung can be reduced by nearly 4.5%,1.7Gy,respectively.The V60 of all the PTV were 95%,and other dose-volume parameters of max dose,mean dose,and HI were 67.474±2.655 Gy,62.568±0.771 Gy,and 12.083±3.686,respectively,for PTV in group G1,compared to 75.142±3.410 Gy,65.161±1.208 Gy,and 20.031±4.351,respectively,for group G2.Conclusions:Correlations between dose-volume parameters of the lung and PTV were observed and indicate that loosening the constraints on maximum and uniform dose in the target volume can reduce the dose to lung effectively.And this will further reduce the incidence of radioactive pneumonia.
