目的 评价血清降钙素原(PCT)、超敏-C反应蛋白(hs-CRP)和白细胞(WBC)浓度在新生儿败血症早期诊断中的临床价值.方法 搜集我院2013年期间新生儿败血症患者共165例(患者个体间无亲缘关系),165例患者均同时进行了PCT、hs-CRP、WBC和血培养检测,所有新生儿败血症患者中血培养结果为判断的金标准.采用Mann-Whitney U检验、ROC曲线和二分类logistic进行统计分

目的 评价血清降钙素原(PCT)、超敏-C反应蛋白(hs-CRP)和白细胞(WBC)浓度在新生儿败血症早期诊断中的临床价值.方法 搜集我院2013年期间新生儿败血症患者共165例(患者个体间无亲缘关系),165例患者均同时进行了PCT、hs-CRP、WBC和血培养检测,所有新生儿败血症患者中血培养结果为判断的金标准.采用Mann-Whitney U检验、ROC曲线和二分类logistic进行统计分析.P<0.05为差别有统计学意义.结果 1.PCT, hs-CRP,WBC的ROC曲线下面积为分别为0.879,0.760,0.632 95%可信区间分别为0.826~0.932,0.675~0.845,0.542~0.724;标准误为分别为0.027,0.044,0.046.2.PCT,hs-CRP,WBC诊断为新生儿败血症的阈值分别为2.165ng/mL,10.800mg/L,6.475× 106[单位],此时约登指数最大(约登指数=灵敏度-特异度),灵敏度分别为0.709,0.724,0.829,特异度分别为0.947,0.737,0.263.PCT,hs-CRP,WBC检测值用于判断新生儿败血症阴性和阳性结果具有显著性意义(P<0.05).结论 PCT,hs-CRP,WBC检测值越大预示血培养阳性的可能性越大,且PCT与hs-CRP检测值越大时特异度较高而WBC的特异度却较低.因此,PCT、hs-CRP与WBC可用于新生儿败血症早期快速诊断.
