Purpose: The aggravated nasolabial fold is one of the important symbols of the aging face, especially the aging "midface".Multiple factors involved in the aggravation of the nasolabial folds, including: skin laxity,the repeated contraction of facial expression muscles, the looseness of zagomatic ligament, masseteric cutaneous ligament, buccal-maxillary?retaining?ligament, the deflation of superficial fat in upper lip, the downward migration of nasolabial fat compartment, the deflation and drop-shaped change of deep medial compartment.Taken the nasolabial folds and peripheral subcutaneous soft tissue as an integratedcomplex, on the basis of anatomic theory of facial fat compartments, this article aimed to introduce a nasolabial folds argumentation technique which couldsimultaneoualyresolve both sag and volume loss in order to retrieve the topography of a youthful face.Method:Multiple layers and multiple injection procedures were applied for nasolabial folds argumentation.The punctiform injection procedure was performed for the argumentation of deep medial fat compartment in order to restore its fusiform shape and form the deep support.The subcutaneous fanlike injection procedure was performed for the upper lip argumentation in order to improve the volume loss of medial inferior area of nasolabial folds.The continuous linear injection procedure was performed through the vertical line from aestheticszygomatic process to nasulabial fold in order to improve the volume loss of midface, lift the sagging lateral superior area of nasolabial fold, and retrieve the curve of a youthful cheek.Result: From February 2012 to January 2013, instructed by the theory of "volume argumentation", the filler, hyaluronic acid (Restylane 2),was injected for 218 patients.The apparent immediate improvement of nasolabial folds and youthful cheek were obtained in 212 cases, partial improvement and satisfaction in 6 cases.The subcutaneous congestion and bruise were found in 7 cases and disappeared after seven days.The best effect sustained from 6 months to 12 months.Conclusion: The argumentation procedures retrieving facial soft tissue volumetric distribution could effectively restore natural and youthful nasolabial folds, to some degree, has a result of midface lift.By accurate understanding the facial anatomy andaging feature of midface fat compartments, with the appropriate filler, the rejuvenation of nasolabial folds could be performed safely, effectively, and rapidly.It could be performed singly or combined with other procedures during the comprehensive facial rejuvenation.
