寰椎侧块-枢椎椎板双皮质螺钉固定的生物力学分析

  目的:分析寰椎侧块-双皮质枢椎椎板螺钉在几种损伤模式下的固定强度、即时稳定性,并与寰椎侧块-单皮质枢椎椎板螺钉、寰椎侧块-枢椎椎弓根螺钉系统固定比较.方法:8例新鲜成人上颈椎标本,分别制作成正常状态、寰椎侧块-枢椎椎板单皮质螺钉固定(C 1LM-C2TU)、寰椎侧块-枢椎双皮质椎板螺钉固定(C1LM-C2TB)、寰椎侧块-枢椎椎弓根螺钉固定(C1LM-C2PS)4组.采用生物材料力学试验机比较3种不同内固定状态下寰枢椎的前屈后伸、侧弯、轴向旋转稳定性.结果:3种内固定后寰枢关节间的运动较完整状态均明显减小(P<0.001).C1LM-C2PS组、C1LM-C2TU组与C1LM-C2TB组在屈伸上无统计学意义(P<0.05),C1LM-C2TB组与C1LM-C2PS组在左右侧弯、左右旋转上无统计学意义,但C1LM-C2TU组在左右侧弯、左右旋转较其他两组明显增大(P<0.05),具有统计学意义.结论:C1LM-C2TB组可以在左右侧弯、左右旋转上提供较C1LM-C2TU组更好的稳定性,与C1LM-C2PS组在前屈、后伸、侧弯及旋转稳定性上无统计学差异.

目的:探讨颈椎动态稳定器在颈椎非融合手术中的安全性及有效性.方法:18例颈椎病患者(21个节段)行颈前路椎间盘切除减压置入颈椎动态稳定器进行非融合手术,术前及术后3d、3个月、6个月进行JOA评分,观察神经功能恢复情况,并摄X射线平片观察椎间隙高度及椎间活动度.结果:经1-6个月随访,JOA平均评分由术前8.2分升至术后15.6分:影像学复查未见颈椎动态稳定器移位、下沉,生理弧度良好,颈椎运动范围

目的:探讨不同手术方式对颈椎骨折伴脊髓损伤的影响。方法:将48例颈椎骨折伴脊髓损伤患者随机分组,分为前后路组给予前路减压、后路固定手术治疗,单纯前路组给予前路减压及固定,单纯后路组给予后路减压及固定,观测疗效。结论:颈椎骨折脱位伴脊髓损伤的前后路手术能更好的充分减压、坚强固定,为脊髓恢复创造条件。
